Information on Referral Request at the Local Vet

Information on consultation at local hospital and relevant forms for SNU VMTH will be provided.

  • Reception Information

    If you wish to request consultation at SNU VMTH, you can make an appointment to the administration office by filling in the medical referral form.

  • Form Information

    Forms from requested local hospitals will also be accepted. Recent test results including imagery/blood tests, if submitted together when requesting consultation, we will take them into full consideration for the patient‘s consultation.

  • Phone Inquiries

    If it is difficult for you to select the relevant department for the patient‘s consultation request, please call the administration office. We will consult the medical staff at each department to help you with the appointment.
